This is not a physical but a spiritual pain, though the body has some share in it -- even a considerable share.

there's a sculpture somewhere in Rome (Vatican, Catholic church, one of the many there) that depicts that very scene. i think that's where you got it from. it also makes a cameo appearance in Angels and Demons if i remember correctly.
